RECORD OF PROCEEDINGS
AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS
IN THE MATTER OF: DOCKET NUMBER: BC-2014-00276
COUNSEL: NONE
HEARING DESIRED: NO
APPLICANT REQUESTS THAT:
The Combat Action Badge (CAB) be added to her DD Form 214,
Certificate of Release or Discharge from Active Duty.
APPLICANT CONTENDS THAT:
She was deployed from 2 Dec 12 to 14 Jun 13, assigned as an
Enroute Critical Care Provider with an Army Aviation Unit flying
Medical Evacuations when an event happened on 28 May 13.
She was awarded the CAB for the event but did not receive the
official paperwork until after her retirement on 1 Nov 13. The
paperwork was not received until 10 Dec 13 and therefore was not
available to be placed in her official records.
The applicants complete submission, with attachments, is at
Exhibit A.
STATEMENT OF FACTS:
On 6 Mar 91, the applicant entered the Regular Air Force.
Per Department of the Army Permanent Special Order Number 344-
038, dated 10 Dec 13, the applicant was awarded the CAB for
personally engaging or engagement by the enemy on 28 May 13.
On 1 Nov 13, the applicant retired from the Air Force.
AIR FORCE EVALUATION:
AFPC/DPSIM states the Air Force is not the subject matter
experts for the Army badges. The Army is held responsible;
therefore, the applicant should forward her request to the Army
Awards and Decorations. If the relief she seeks is not to her
satisfaction she may resubmit the DD Form 149 to the Army Board
for Correction of Military Records, which is the appropriate
forum in making a determination of relief through correction of
a military record.
The complete DPSIM evaluation is at Exhibit C.
APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the
applicant on 19 Nov 14 for review and comment within 30 days
(Exhibit D). As of this date, no response has been received by
this office.
THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice. We note
DPSIM states the Air Force is not the subject matter experts for
the Army badges and advises the applicant to forward her request
to the Army Awards and Decorations Branch. However, based on
Permanent Order Number 344-038, dated 10 Dec 13, she was already
awarded the Combat Action Badge. Therefore, we find the
evidence presented sufficient to recommend her DD Form 214 be
amended to include the Combat Action Badge. Accordingly, we
recommend the applicants records be corrected as indicated
below.
THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air
Force relating to APPLICANT, be corrected to show that her DD
Form 214, Certificate of Release or Discharge from Active Duty,
issued in conjunction with her 1 November 2013, retirement, be
amended to include the Combat Action Badge.
All members voted to correct the records as recommended. The
following documentary evidence pertaining to AFBCMR Docket
Number BC-2014-00276 was considered:
Exhibit A. DD Form 149, dated 17 Jan 14, w/atchs.
Exhibit B. Applicant's Master Personnel Records.
Exhibit C. Memorandum, AFPC/DPSIM, dated 19 Nov 14.

This extension was completed, signed, and approved in Mar 2009; however, the local personnel office said they could not update the system until 1 Dec 2009, which was the first day of her extension. In this regard, we note that DPSIM initially recommended disapproval of the applicants request; however, after reviewing the extension paperwork, DPSIM now recommends 17.5 days of leave be restored to her leave account and we agree. ...
